You may enroll with just your school ID or original birth certificate. Lacking requirements can be submitted at a later date

INCOMING FIRST YEAR COLLEGE

  • Notice of Eligibility for Admission (to be received through email once you have filled up the enrollment application form)
  • Original Certificate of Good Moral Character from school origin
  • Original PSA-Authenticated Birth Certificate
  • Original Uncancelled Grade 12 Report Card (Form 138)
  • Photocopy of Certificate of Completion or Diploma
  • 4 pcs of identical 2×2 photos with name plate and white background
  • Data Privacy Policy*
  • Undertaking Contract*
    Promissory Note* – In case of lacking requirements, student may enroll with only school ID or original PSA birth certificate. We’ll provide a signed promissory note for any missing documents (form included).

INCOMING GRADE 11

  • Notice of Eligibility for Admission (to be received through email once you have filled up the enrollment application form)

  • National Career Assessment Examination Result (NCAE) (if available)

TRANSFER STUDENT / SECOND COURSE

  • Original Transfer Credentials/Honorable Dismissal
  • Certificate of Eligibility to Transfer from the school last attended

The first step of enrollment for Transfer Students or Second Course takers is Grade Evaluation. Please send us a copy of your TOR for Evaluation so our Program Chair/Dean can evaluate your grades and standing. This step can help you identify what subjects will be credited.

APPLICATION ATTACHMENTS

  • Letter from student’s parent or guardian stating the reason for withdrawal/dropping of subjects addressed to the Registrar’s Office along with supporting documents such as certifications or records (e.g., medical certificate for medical reasons)
  • Photocopy of valid ID from parent/guardian.
  • To be submitted to the Admission’s Office for screening of application. (Monday to Friday, 8:00 AM to 4:00 PM)
ADDING/CHANGING OF SUBJECTS
– Applicable for students who wants to add subject based on the allowable units required in the curriculum or change a particular subject/schedule for conflict/dissolved classes.

Instructions:   Fill-out the form completely. Proceed to respective department or colleges concerned for interview and signature. Please note that only those fully accomplished form will be accepted for processing.

TERMS AND CONDITIONS

All applicants are requested to read carefully and take note of the following terms and conditions for Admission and enrollment of incoming or new students for the 1st Semester of the academic year 2024-2025:

  1. The applicants must conform to the onsite enrollment guidelines and requirements of LPU-DAVAO, including the schedules and deadlines, if and when the school declares them.
  2. All applications are subject to review and approval upon submission of the necessary requirements.
  3. The registered section, along with the class schedule, is determined by the school system and cannot be chosen by the applicant.
  4. Enrollment is considered official only upon the applicant’s complete and accurate compliance with the following:
    • Presentation and submission of original/photocopy admission and enrollment requirements to LPU-DAVAO before the start of classes.
    • Completion of enrollment procedure set by the institution within the same period (please see enrollment procedures).
  5. Withdrawal Dropping of Subject/s (part of load/entire load) should be filed to the Registrar’s Office subject to the existing policies and procedures. 
  6. This Enrollment Procedures are applicable only for 1st Semester, Academic Year 2024-2025.
  7. All applicants should strictly follow these terms and conditions including the Data Privacy Agreement as stated in the Applicant Data Entry. Any unlawful or prohibited use and act thereof, which constitute a violation of the Admission and Enrollment shall be a ground to declare the admission and enrollment application null and void.
